This form is for current and former students to request only undergraduate student conduct or disciplinary records to be released to others or themselves.

Upon written request and consent from the student, student conduct records will be reported to other institutions when applying for transfer, applying to graduate or professional schools, applying to Federal, State or Local Bar Associations, the Federal government, or in the course of any employment or governmental background check.
There are two types of Requests:
1) a letter from the Dean of Students with student conduct information as follows: incident date, responsible conduct charges, sanctions, and sanctions completion info.
2) a form from another agency (the requestor should attach the form at the end of this request).
Your Request has to be reviewed by our office once it is submitted and it will take 7-10 business days to complete.
